This will occur if a questions has been missed from a page.
Please review the page & navigate to the question highlighted in red to answer it. You may need to scroll up or down to locate the question.
When the questions has been answered the submit button will activate.
This may also occur if the user is starting the questionnaire, answering a few questions and leaving it open in a tab on their browser, while they navigate to another browser page.
If this situation occurs the user should log out of that session using the log out option, close the window and click on the questionnaire link to recommence the questionnaire.
